@Christoph Agten The pt did knee and hip Mri , Xray of all right extremity which was unremarkable. The pt on pregabline 75 BID with no improvement Straight leg raise test + MRI lumbar show: - mild disc bulge cause mild narrowing on both lateral recess.annular fissure at the same level -pseudoarthrosis Plan: steroid injection for diagnostic purposes (I will treat patient symptom, not Mri finding)

Totally agree I recommend the patient for lab rheumatological factors and sacroiliac joint MRI.
